Patient Registration

You will be asked to complete and sign a registration form.  On registering, the receptionist will ask you to make a new patient check appointment with the Health Care Assistant.  This is purely voluntary and we would recommed that you do this as your records may not be received by the surgery before your first visit to the doctor, therefore the doctor will have no previous history of you. 

There is disabled patient access to the premises.
 
If you are over 75 and have not been seen in the last year, or between 16 and 75 and have not been seen in the last 3 years, you may request a consultation with our Health Care Assistant.
